Care Plans
The GPs at Doctors @ Morningside Brisbane create and manage Care Plans. A care plan is a structured plan created by a general practitioner (GP) to help people manage ongoing or complex health conditions with support from other allied health professionals.
In Australia, it is funded through Medicare, which can reduce the cost of allied health services. With a Medicare billed Care Plan, patients receive rebates from allied health services.
If you have a GP Medicare Care Plan (often called a Chronic Disease Management plan or GPMP/TCA):
- You can get a Medicare rebate for allied health visits (like physiotherapy, psychology, dietetics, exercise physiology).
- The rebate is a set amount per session, not the full cost.
Important limits
- Usually only up to 5 allied health sessions per year are covered under a care plan
- You must be referred by your GP
- The rebate only applies if the provider offers Medicare billing
